Maternal Health Resource Bundle

What is the Maternal Health Resource Bundle?

The Maternal Health Resource Bundle is a collection of resources for providers and their patients in Federally Qualified Health Centers as well as other providers of maternal health care. The purpose of the Maternal Health Resource Bundle is to provide a compiled list of recently developed resources to promote the health of pregnant and postpartum persons.

Resources for Providers

The ATD system allows hospital and MCO providers to connect in real time with information on patient’s admission, transfer, and discharge from hospitals. When a patient is either seen in the emergency room or is admitted for the pregnancy/birth, a notification is received by the FQHC if they are participating in the HFS HealthChoice Illinois ADT Program. The participating FQHC will also receive scheduled reports with this information.

Included in this Maternal Health Resource Bundle are PowerPoint slides that were created for an Illinois Department of Healthcare and Family Services (HFS) webinar on the ADT System, which can downloaded below.  We also encourage all sites to fill out this survey to enable your site  to receive an ADT on-boarding package and start the implementation process to use the ADT FQHC System.

I PROMOTE-IL, with support from the Illinois Perinatal Quality Collaborative (ILPQC), has developed an Implicit Bias Training Inventory, in which potential implicit bias focused trainings that are most appropriate for those who care for pregnant and postpartum persons and their families have been included.  We developed this resource for potential use by health care providers in response to Illinois Public Act 102-0004, which requires implicit bias awareness training for health care professionals’ license or registration renewals beginning January 1, 2023. The Illinois Department of Healthcare and Family Services (HFS) has created a Ready to Renew Messaging Toolkit to assist providers and others in helping Medicaid patients with redetermination now that the Public Health Emergency has ended. We have included the English version of the Illinois Medicaid Ready to Renew Toolkit below, which includes flyers, palm cards, and language for social media that providers can share with patients about preparing for the patient’s renewal date. The Illinois Association for Medicaid Health Plans (IAMHP) created a Maternal Health Toolkit for Providers, to serve as a guide to maternal health benefits in Illinois. The Toolkit was updated in May 2024 and can be downloaded below.

Resources for Patients

The Chicago Fetal and Infant Mortality Review (FIMR) Community Action Team, in partnership with the Illinois Association of Medicaid Health Plans (IAMHP), has created three Toolkits for patients on Medicaid:

  1. Illinois Medicaid Managed Care Maternal Health Toolkit. This Toolkit serves as a starting point for Medicaid MCO Expecting Members to better understand their benefits.
  2. Illinois Medicaid Managed Care Maternal Mental Health Toolkit. This Toolkit is a guide on how a patient can select a doctor covered by their plan, learn more about supports covered while pregnant, after pregnancy, and for their family, and learn more about covered treatments, including therapy and medication.
  3. Illinois Medicaid Managed Care Consumer Benefits Transportation Toolkit: This Toolkit is a guide focused on how patients can set up transportation, what additional rides are covered by patients’ health plans, and what to think about when planning transportation ahead of time.

Maternal Health Digital Library Business Cards were created by I PROMOTE-IL to provide pregnant and postpartum patients and those who serve them access to I PROMOTE-IL’s Digital Resource Library. The Digital Resource Library includes tips and resources for a safe pregnancy and recovery, infant care, birth control, vaccines, telehealth, and more. The Business Cards can be downloaded here.
